An experienced bowhunter, Gavin Heale was practicing for a hunt in the South African wilderness. He drew back on his bow, and the arrow shattered. Half of the arrow flew harmlessly out into the grass. The other half impaled his hand. He was 400 miles from home.

Always flex your arrows to make sure there is no damage to them or this could happen. Things like this can occur when you either shoot an arrow with cracked carbon fibers (which reduce the spine - the stiffness of the arrow) or shoot an arrow that is underspined for the draw weight of the bow. The arrow flexes too much and cracks, sending the back half off the left left for right handed shooters, which just happens to be where your hand is holding your bow (and for left handed shooters, the arrow breaks to the right so you don't fare any better!)

Sometimes, this can happen when a person puts on a much heavier broadhead than they were shooting before and the heavier broadhead weakens the spine of the arrow shaft.
