Transit through the Netherlands - Form clarity questions

Good day all,

I am trying to complete the application consent for weapons and ammunition documents for a trip in Spring of 2025. The instructions are a little ambiguous as to what they are looking for.

On sections 4, 5 and 6. Who is the applicant, consignee and addressee? I figured I would list myself under each but should I list the address of the outfitter I will be staying with under section 6?

Under section 7 it is asking me for Weapons/Ammunition info. What are they wanting in section 7a (number) ? If I am bringing two firearms, is this simply #1 and #2 or do they mean N\number of applicable pieces. So I would have 1 in each of those boxes for each of my two firearms?

Regarding ammunition, how am I to list that within the annex? Do I list the number of individual rounds under number? What do I list for manufacture number?

I tried to find an example of the form online to help answer the question but no luck. Hoping I can chat briefly with someone here or see a completed section. I am travelling from Canada to Amsterdam and then onto JNB.

I assume I would send two sets of this paperwork (Travel to and return trip) to the customs administration.

What else could I be missing?
